Support hospital discharge, allow more timely and preventative care
Costs of treatment are greatly reduced by early detection of deterioration
Improve clinical workflows, reduced hospital readmission
Patient outcomes are greatly improved if deterioration is detected and acted on
For many patients there is a continuing need to be monitored after leaving hospital. ARMED’s wireless monitoring system allows ICU-grade patient data to be collected and reported beat-by-beat from virtually anywhere, keeping patients in constant touch with their clinician post discharge.
The patient is provided with the ARMED monitoring device which is worn around the upper left arm. The system is pre-configured enabling an NFC tap for very quick or remote deployments. The device contains a sim card which passively forwards data into the ARMED platform without the need to do anything.
Supporting a “Discharge to Assess Model”, there is the ability for key health stakeholders supporting and monitoring an individual patient's needs to assess individual remote monitoring info. The team can include consultants, GPs, nurses and other physio and OT specialists including social prescribing and dieticians.
With all learnings captured and documented within the ARMED portal. Custom forms can be created for an individual patient or for an entire patient group to fill out periodically, providing much-needed context regarding a patient's health.
Data is continously collected by ARMED's powerful 'AI' platform, which analyses against models of care to provide ongoing reporting to all relevant stakeholders and alerting when pre-set parameters have been breached.
The system periodically reviews the state of the patients health, assessing outcomes against plan parameters and notifying when conditions are stable.
ARMED provides a full range of care solutions and devices allowing for a more independent life after recovery with less chance of any readmission to hospital.
Our ARMED system uses powerful predictive algorithms to analyse a constant stream of data to forecast potential risks to users, prior to any adverse events even happening.
All alert notifications and other important information is shared with any relevant health professionals, care providers, patients and their families through a clear and easy to understand dashboard.
ARMED one of the UK’s leading innovators in supporting early intervention and preventive care, providing actionable insights derived from a wide range of vital sign data collected via wearables and in home technology.
