For many patients there is a continuing need to be monitored after leaving hospital. ARMED’s wireless monitoring system allows ICU-grade patient data to be collected and reported beat-by-beat from virtually anywhere, keeping patients in constant touch with their clinician post discharge.
The patient is provided with the ARMED monitoring device which is worn around the upper left arm. The system is pre-configured enabling an NFC tap for very quick or remote deployments. The device contains a sim card which passively forwards data into the ARMED platform without the need to do anything.
Supporting a “Discharge to Assess Model”, there is the ability for key health stakeholders supporting and monitoring an individual patient's needs to assess individual remote monitoring info. The team can include consultants, GPs, nurses and other physio and OT specialists including social prescribing and dieticians.
With all learnings captured and documented within the ARMED portal. Custom forms can be created for an individual patient or for an entire patient group to fill out periodically, providing much-needed context regarding a patient's health.
